Holland Community Foundation Applications Instructions

Please submit the attached Grant Application Cover Sheet with the following information. Include the questions with your responses.

PROGRAM

I-A. Describe the program for which you are seeking funding and its basic objectives.

I-B. What is the need for this program? Use data specific to Holland, wherever possible. How many people will it serve?

I-C. Describe the evaluation plan you will use to determine if the project's goals have been met.

I-D. General objectives of your organization. How many individuals or families are served by your organization annually?

FINANCIAL

II-A. What is the amount being requested? What is the total cost of the project? What is the specific purpose of funds requested in this proposal?

II-B. Include a detailed budget for the project, including price quotes for any equipment.

II-C. When will these funds be needed?

II-D. Explain how the project will be funded beyond the time period of this request?

II-E. Indicate other sources of funds that are or may be available for this program and the current status of these proposals.

II-F. If this request for funding is not approved, what alternative plan would you probably follow?

ORGANIZATIONAL INFORMATION

III-A. Names and affiliations of members of the Board of Trustees or Governing Board.

III-B. If your organization has previously applied for grants from the Holland Community Foundation, please list the project, the year, and the amount funded or denied.

III-C. Include your current organization operating budget.

Please submit one copy only of the following:

A. Review or audit letter from the organization's most recent audit.

B. A copy of the grant applicant's most recent letter of exemption under Section 501 (c)(3) of the Internal Revenue Code.

C. Letters of support. If included, they should be used to verify project need and the ability of your organization to achieve the project goals.

D. Letters of cooperation from other organizations, if appropriate to your request.

Please Note:

Completed applications must be received by January 15, 1996. Please provide ten three-hole-punched copies of your application. If the project is youth related please provide twenty-five copies. Do not use binders, covers, or folders. Grants are announced in April.

It is not possible to approve all requests for funds. The denial of a request does not necessarily imply that the proposal is not worthwhile or that the organization should not apply for funds for another proposal in the future. All applicants will be informed of the disposition of each request after the trustees have considered the Distribution Committee's recommendation.
